A care management platform for CLCM that coordinates services, holds patient records and handles the day-to-day admin. Coordinators can manage schedules and track how patients are progressing.

CLCM needed to coordinate care across providers, keep patient information in order and reduce the manual admin behind service delivery.

We built the system on WordPress with care coordination tools, patient tracking, scheduling and reporting.
